When do doctors suggest IVF?

Doctors usually suggest IVF when simpler treatments have not worked, or when test results point to it from the start. The common reasons are:

Blocked fallopian tubes. If the tubes are blocked, the egg cannot reach the uterus. IVF works around the tubes completely, because fertilisation happens in the lab.

Low sperm count or poor sperm movement. When there are too few healthy, moving sperm for natural conception, IVF with ICSI can be used. In ICSI, one sperm is placed directly inside the egg. Read about male fertility tests

Endometriosis. Mild endometriosis often responds to medicine or surgery. If pregnancy still does not happen after that, the doctor may suggest IVF. Read about female fertility problems

Problems with ovulation. When eggs are not released regularly, doctors usually try tablets or injections with IUI first. If that does not work, IVF is the next step. Read about IUI

Age. As a woman gets older, both the number and the quality of eggs fall. Doctors often suggest not waiting too long before moving to IVF, and may add ICSI.

No clear cause found. Sometimes all the tests come back normal and pregnancy still does not happen. This is called unexplained infertility. IVF is one of the options a doctor may discuss.

How does IVF work, step by step?

1. Consultation and tests. The doctor looks at both partners’ history and reports. Blood tests, a scan for the wife and a semen test for the husband show what is causing the problem and what treatment suits you.
2. Injections to grow eggs (about 8 to 14 days). In a normal month the body releases one egg. Injections help the ovaries grow several eggs instead. The injections are given under the skin with a very thin needle.
3. Scans to watch the eggs. Short ultrasound scans every few days show how the eggs are growing. The doctor adjusts the medicine dose based on what the scans show.
4. Trigger injection. When the eggs are ready, one final injection is given at an exact time. Egg collection is planned for about 34 to 36 hours later.
5. Egg collection (5 to 10 minutes). The eggs are collected with a thin needle guided by ultrasound. This is done under light sedation, so you are asleep. Most women go home the same day.
6. Sperm sample. The husband gives a semen sample on the same day, unless frozen or donor sperm is being used. The sample is washed and prepared in the lab.
7. Fertilisation in the lab. The eggs and sperm are brought together. In ICSI, the embryologist places one sperm directly into each egg using a fine instrument under a microscope.
8. Embryo growth (3 to 5 days). The embryos are kept in an incubator that holds the right temperature and air. The embryologist checks how they are growing each day.
9. Embryo transfer (2 to 5 minutes). An embryo is placed into the uterus through a thin soft tube. No anaesthesia is needed. You rest for 15 to 20 minutes and then go home.
10. Pregnancy test. A blood test about two weeks after the transfer shows whether the treatment has worked.

How long does IVF take, and how many visits are needed?

One IVF cycle takes about three weeks from the first injection to the embryo transfer, plus a two-week wait for the pregnancy test.
